In the event that one or more of your pets has need for medical treatment, or has a medical emergency while you are away, (example: injury from rough dog play, torn toenail, laceration) we need the authority to take your pet to our veterinarian, your veterinarian, or to the  emergency clinic. 
We will make every attempt to contact you at all of the emergency numbers you give us – but, if we cannot reach you in a reasonable amount of time, we will do what we feel is necessary for the health of your pet.
  We have access to the veterinarians at Crossroads Veterinary Hospital in Woodstock, and also know the locations of Cobb Emergency Veterinary Hospital and Cherokee Emergency Hospital.
If we can get to your personal veterinarian, we will do so. We ask that you call your personal veterinarian prior to leaving to make payment arrangements and give them verbal or written permission for us to act as agents for your pet(s). This could save precious moments in case of a serious injury.

    By your signature below, you agree to be monetarily responsible for any debt incurred involving any and all medical treatment of your pet(s). Further, you release DogDaze Pet Care and its employees and agents from any responsibility or liability for claims damages or debts related to such injury or medical treatment, including transportation to and from the veterinary clinic.
